Dr. Gilad Bar Dayan explains that using a laser allows him to perform a series of dental treatments swiftly and efficiently, allowing the patient to enjoy a pain and anxiety free dental appointment

The field of medicine in general, and particularly the field of dentistry, is seeing technological innovation. However, it seems that when it comes to using laser devices for dental treatment, many dentists in Israel have not yet made the leap to using the existing technologies even though dental laser treatments have been around for about twenty years.
Dr. Gilad Bar Dayan, a dentist since 2000 and a graduate of the Hebrew University of Jerusalem who won an award for his outstanding doctoral thesis, has been combining laser treatments alongside conventional treatments in his private practice in recent years. He received a Laser Mastership degree from Aachen University in Germany, where he learned the intricacies of working with laser devices. In his private clinic he uses a variety of advanced laser devices used for different types of dental treatments of varying complexity.
"The patient hardly feels anything"
Using the minimally invasive treatment a wide variety of dental treatments can be performed, from simple operations to more complex treatments. "For example, if we take the most basic dental treatment, a filling, you can already see the benefits of using the laser," explains Dr. Bar Dayan.
He adds that "in the process of performing a filling (or as it is commonly known today – restoration) it is customary to drill into the tooth, which causes significant vibration and noise that many people find very disturbing. On the other hand, laser treatment is carried out without any contact. Of course there is no vibration at all. The noise produced by the device is different, and is usually considered to be quite reasonable. Since there is no contact, the experience as a whole is completely different from conventional dental care. The patient hardly feels anything, and it is viewed as a more pleasant experience. The treatment can be performed under local anesthesia or without anesthesia at all, according to the patient's preference. Another advantage is that the laser does not incise the tooth. The light beam vaporizes tooth material in a precise manner and the area to which the laser is directed will be completely disinfected of bacteria."
"You can use a laser to whiten teeth quickly"
Another example of a common treatment that can be performed using the laser is with periodontal disease. Dr. Bar Dayan points out that about a third of the world's population suffers from this disease, which is a receding of the jawbone that can cause the teeth to wobble and eventually fall out.
“The basic treatment for this is root planning," he explains. "It's like having the dental hygienist clean plaque, but below the level of the gums and under local anesthesia. Using the laser, instead of injecting antibiotic or antiseptic substances into the gums as is customary in many cases, most of the bacteria can be destroyed with the help of light energy and get the same level of effectiveness as with the antibiotic treatment."
As Dr. Bar Dayan points out, the use of laser devices is also effective in more complex dental treatments: "For example, if a root canal is needed, when we treat with a laser the disinfection is performed in a particularly efficient manner. If we compare conventional root canal treatment with treatment that uses light energy, it seems that in conventional treatment the disinfection is performed through a combination of mechanical friction of devices on the inner wall of the root canals and chemical rinses, mainly rinses containing chlorine. The mechanical friction is required to remove the inner layer of the root canal wall, where the bacteria live. This harms the survival of the tooth over the years and encourages the formation of cracks."
He further explains that "a root canal treatment that uses light energy creates bubbles that advance rapidly in the root canal system and remove organic substances, bacteria and tooth particles that interfere with the treatment. Those bubbles enable, on the one hand, very effective removal of the microorganisms living in the root canals, and on the other hand, the traditional expansion is carried out in a minimally invasive manner so as not to encourage cracks to form.”
“Even in the case of a tooth extraction, for example, it is necessary to clean the inflammation at the bottom of the tooth. The conventional method is to use a kind of small spoon that scrapes the inflamed tissue. With the help of the laser, the inflamed tissue can be removed in order to reach the clean bone. It also helps a lot in the healing phase, which is faster thanks to the laser. Today we are in an era where many patients have received implants that serve as a basis for oral rehabilitation. Some implants are damaged due to periodontal disease."
According to him, in the traditional treatments for failed implants it is difficult to remove the layer of bacteria from the implant without damaging the implant itself. The laser beam is aimed at the implant and vaporizes the bacteria without damaging the implant itself and without scattering microscopic particles from the implant inside the tissue. The result is much better healing and a higher chance of success.
The goal: to save money and treatments
As mentioned, the field of laser dentistry in Israel is not particularly widespread, and Dr. Bar Dayan explains why: "We are not a large group of dentists in Israel who are involved in this, and I think this is for two reasons. The first reason is that it is not taught here in the university, which I believe will change. The second reason is the high cost of training abroad and of the equipment. Despite this, it is important to understand that the treatments themselves are not more expensive and sometimes may even be less. I do not price treatment with laser differently compared to the pricing of conventional treatment. The goal of laser treatment is to save money, not to add more."
It turns out that in many cases, laser treatment of deep caries can save the need for root canal treatment, followed by expensive restoration. This means that using laser shortens the number of treatments required and makes it possible to perform treatments in different areas of the mouth during the same visit.
In his private clinic, Dr. Bar Dayan uses three different laser devices, each of which is intended for a different type of dental treatment. Recently he started using a particularly innovative and advanced device. "There is a device that is actually used to treat the hard tissues, bone and teeth. Other indications include treatment of trapped teeth that need to be exposed, various surgeries that are easily and delicately performed using the laser, and more," he explains. "Another device can stop bleeding, promote healing, treat aphthae and herpes sores, and more.”
"During treatment I combine the various laser devices. Almost all of the treatments at the clinic are performed in combination with the laser devices. The clinic is very advanced and offers a wide range of care and state-of-the-art devices, including a 3D scanner that can scan the teeth and transfer information to the laboratory in a computerized manner, instead of the previous method of taking impressions. Local anesthesia is performed using a computerized syringe with minimal sensation and with maximum consideration for the patient."
Also effective for aesthetic treatments
A field that has been gaining momentum in recent years is aesthetic treatments, and here, too, the use of laser devices offers major advantages. "Wrinkles can be filled without injection by using a laser," says Dr. Bar Dayan. "Hyaluronic acid is used for aesthetic injections. I do that too, but with the laser, the patient's own collagen is created and thus the treatment lasts for a longer time.”
There are numerous uses for the laser device in the field of aesthetics. "You can use a laser to whiten teeth much faster, and the color of the tooth remains for a longer period of time. Another treatment is to level the smile line when there are gums at different heights. The laser device makes it possible to achieve a beautiful and symmetrical appearance," he explains. "Some people are not satisfied with the dark color of their gums, and sometimes even avoid smiling because of it. Using a laser, you can achieve very impressive results where the gums become nice and pink."
