You should look to a map of Europe before and after WWII. When a nation wages offensive war and loses, it can lose territory. At the end of wars borders are often reconfigured. The areas are "disputed" because there never was a Peace Treaty signed. Borders are formed as part of Peace Treaties, not armistices. The lines after fighting has stopped due to a cease fire are temporary until treaties are negotiated.
