I assume Jeremiah and Rambam are speaking of a milchemes mitzvah or 'commanded war' such as protecting the Holy Land from Amalekites. Without going into detail it appears that Hamas and Hezbollah come into the category of Amalekites, whose goal is the destruction of Israel. In such a case even if the enemy operates from civilian locations one must defeat them and in the case of Hamas the people themselves actually voted for a Hamas govt and therefore share some of the responsibility. When Israel left Gaza in 2005 the PA should have commenced state building, but instead Hamas created a terrorist state and a front for Iran.
