when the army said that there were mistakes, they are talking about the times where no terrorists were involved and there was a mistaken identity, or coordinates.
the rest would be pertaining to people that are collateral damage. so when they killed a terrorist, or targeted a terrorist, an a civilian was killed, they would fall under that situation
