MAD only works when all players are more concerned with their lives than their enemies` deaths. It worked well with the Soviet Union because they were declared aethiests, concerned with this life and not the afterlife.
For similar reasons MAD may work with the current autocratic Arab regimes. The biggest problem is that they are all at risk to local religious rebellion of the same culture that encourages violent martyrdom (i.e. suicide bombing).
